SQL Lookup not appearing after the solution is deployed

Solution/Immediate Workaround

Check whether the look up controls which are not displayed in form has property set for "when field changes".
This is expected behaviour. If you set the lookup control property "when field changes" as blank, then the look up will be visible in the form at run time. Refer attached screenshot ("lookup.png").

In Foundation/evolve this is how it is designed. It wouldn't impact Business Users or the Testing; you just need to check out the Process at what staged it should be visible and what it shouldn't ; if you require it to be visible all the time then you would need to have that option checked.
